Job Description
Key Responsibilities: Patient Assessment and Care Planning: Conduct comprehensive assessments of patients" mental and physical health. Develop and implement individualized care plans in collaboration with the healthcare team. Monitor and evaluate patients" progress and adjust care plans as needed. Medication Management: Administer medications as prescribed by physicians. Monitor patients for side effects and efficacy of medications. Educate patients and their families about medication management and adherence. Therapeutic Interventions: Provide therapeutic interventions, including counseling, crisis intervention, and behavior management. Facilitate group therapy sessions and psychoeducational activities. Support patients in developing coping skills and strategies for managing their conditions. Patient and Family Education: Educate patients and their families about mental health conditions, treatment options, and self-care. Provide resources and support for families dealing with mental health challenges. Collaboration and Communication: Collaborate with psychiatrists, psychologists, social workers, and other healthcare professionals to provide comprehensive care. Communicate effectively with patients, families, and the healthcare team. Participate in multidisciplinary team meetings and case conferences. Documentation and Compliance: Maintain accurate and timely documentation of patient assessments, care plans, and interventions. Ensure compliance with all regulatory and institutional policies and procedures. Participate in quality improvement initiatives and contribute to the development of best practices. Professional Development: Stay current with advancements in psychiatric nursing and mental health care. Participate in continuing education and professional development activities. Mentor and support new staff and nursing students.
