As a Nurse in the Chronic Patient Care team at Cloudphysician's Clinical Capability Center (CCC), your role is crucial in providing exceptional healthcare to patients with chronic conditions. You will act as a bridge between patients and healthcare providers, ensuring seamless communication, accurate information exchange, and coordinated care to improve patient outcomes. Your responsibilities will include addressing clinical queries, managing patient concerns, and offering continuous support to ensure optimal care delivery. Your duties will involve ensuring timely and precise placement of medication orders in the pharmacy, based on accurate dosage calculations, strength, and quantity requirements as per the provider's prescription approval. You will collaborate with healthcare teams, including US-based doctors and nurses, to evaluate and ensure continuity of care for patients. Maintaining effective communication with US providers to facilitate the smooth transfer of patient information and ensure care coordination will be a key aspect of your role. Adhering to US nursing standards, protocols, and regulations, including compliance with HIPAA and other legal frameworks for patient confidentiality and privacy, is essential. Engaging in therapeutic communication with patients through telemedicine platforms, addressing their queries, and providing regular updates on their care will be part of your daily tasks. Additionally, you will participate in training programs to stay updated on Cloudphysician's nursing protocols and the latest US healthcare standards, contributing to quality improvement initiatives and assisting in the onboarding and mentorship of new team members. The ideal candidate for this position should hold a Bachelor of Science in Nursing (BSN) or Master of Science in Nursing (MSN) and possess a minimum of 6 months to 1 year of hands-on experience in reputed hospital ICUs or wards. Experience with telemedicine, proficiency in Electronic Medical Records (EMR) systems, or remote patient monitoring is preferred. You will be based in Bangalore, providing remote support for US patients, and will work in rotational shifts, including night/day shifts, weekends, and holidays, to align with US time zones and operations.
