Conduct psychosocial assessments upon admission to identify clients" mental health history, strengths, risks, and support needs. Develop and implement individualized care and recovery plans in coordination with clinical and support teams. Facilitate individual, group, and family therapy sessions using evidence-based approaches. Monitor and document residents" progress, behavioral changes, and treatment responses.
